Broadman, Welcome Visitor Form WV-7, Set of 100
During a busy church service, your pastor may not have the opportunity to meet with guests or every member who may have an issue to discuss. These pastor contact cards are ways to create a list of everyone who needs a follow up conversation, or information updated in the directory. The detachable card at the top says:

The reverse side has the message:
We pray that you may
receive a rich blessing in this period of worship
and that Christ the Lord will be more real to you
because you have worshipped here today.
Encourage the guest or church member to fill out the information card with the date, name, address, phone number, city, state, zip code, business, home church. Then check an age group, and answer if they would like a visit from the pastor. The reverse side has space for the pastor including name, address, phone number, and check marks to decide if the person is sick, a new resident, desiring a call form the pastor, wanting offering envelopes, is a prospective member, or has changed to a new address.
With a set of 100 of these cards, you can keep them at the pews, near offering envelopes, or on a welcome table. Guests and members can fill out the card as needed, and drop in the offering plate for follow up contact or updates.
